Self-Management Support for people after stroke and their families
Welcome
People who had a stroke, their family members and professionals in stroke rehabilitation share their experiences and advice to give you hope, confidence and strategies to live the life you want.
This platform was developed in partnership between the South Eastern Health and Social Care Trust in Northern Ireland and the social enterprise Bridges Self-Management, which is an expert in evidence-based approaches to co-production and self-management support.
